Application Process
1. Register with COGENTS Systems to complete your Complete Background Check prior to submitting your application for licensure.

2. Mail in completed, signed and notarized application for Licensure by Endorsement including all of the following documents: – Application fee (check or money order) – Official college transcripts – Verification of original licensure you obtained by examination of another state
• If the state participates in Nursys, you must complete the Nursys verification process, pay their fee and designate Georgia as the receipt of the verification.
• If the state does not participate in Nursys, you must contact their state Board directly for further instructions. They will need to submit an official Verification of License form to the Georgia Board of Nursing on your behalf. Verification of your current licensure in another state (not app licable if you are currently licensed in the state of your original license)
• If the state participates in Nursys, you must complete the Nursys verification process, pay their fee and designate Georgia as the receipt of the verification.
• If the state does not participate in Nursys, you must contact their state Board directly for further instructions. They will need to submit an official Verification of License form to the Georgia Board of Nursing on your behalf. Enclose secure and verifiable documentation of United States cit izenship or legal immigration status. Submit one of the following:
• Verification of employment documenting (500) hours of licensed practice within the four (4) years immediately preceding the date of application.
• Proof of graduation from an approved nursing education program within four (4) years immediately preceding the date of application.
• Completion of a Georgia Board of Nursing approved
Renewal License
Renewal of your license occurs every 2 years
• Licenses expire January 31 of even or odd numbered years
• You can renew online or mail in an application
• Renewal fee: $65.00 Required to complete 20 hours of continuing education hours You can satisfy this requirement by selecting one option from the following five options.
Important Things to Know About Applying for Licensure for Endorsement
• To avoid processing delays please be sure to include all required documentation with your application packet.
• Applications are valid for one year from the date of submission.
• When mailing your application please use a 9×12 envelope and do not fold or staple any of the documents.
• The Board of Nursing does not provide a specific form to use when requesting licensure verification from states that do not participate in Nursys.
• If you already have a Single State Georgia License and you want to upgrade it to a Multistate License and you meet the requirements, fill out this application.
