This position offers day shifts from 08:00 to 16:30, with 4-5 weekend shifts required in a 6-week period.

Job Description

As a Registered Nurse Case Manager, you will be responsible for managing the care of up to 25 patients on weekdays and 45 patients on weekends. This role requires collaboration across interdisciplinary teams, including Physical Therapy and Social Services, with a focus on Discharge Planning and Care Coordination. You will utilize the Epic charting system for documentation.

Responsibilities

• Conduct patient assessments and determine care coordination needs.
• Facilitate family meetings and participate in the treatment planning process.
• Manage Discharge Planning processes and ensure compliance with CMS guidelines.
• Handle prior authorizations and arrange for DME, SNF, and ambulance transport as needed.
• Support patients with complex needs including mental health, substance abuse, and social challenges.

Required Experience

• Minimum of 2 years of acute care case management or discharge planning experience.
• Experience working in an acute care hospital setting; behavioral health experience does not apply.

Certifications/Licensure

• Active California state nursing license required.
• Pending licenses accepted, but candidate must be licensed upon start date.
