Required to work every other weekend. Day Shift from 8:00 AM to 4:30 PM.

Job Description

The Registered Nurse will manage cases within an acute care setting, coordinating patient care and ensuring consistent discharge planning. The nurse will utilize Epic for charting and collaborate with interdisciplinary teams. Opportunities for outdoor activities and housing assistance are available through the facility.

Responsibilities

• Coordinate care for assigned patients, ensuring adherence to admission criteria and discharge planning.
• Conduct needs assessments and manage prior authorizations.
• Facilitate communication with patients, families, and interdisciplinary teams.
• Maintain compliance with CMS guidelines and HIPAA regulations.
• Handle appeals, denials, and benefit eligibility inquiries.

Required Experience

• Minimum of 2 years of acute care experience.
• Recent experience within the last year in case management.

Certifications/Licensure

• Valid RN license required.
• Must have a compact license in hand at submission.
