ARMStaffing is looking for a Registered Nurse, Case Manager (Day Shift) in Apple Valley, California.

This is a 13-week contract position.

Schedule/Shift

Day Shift: 0800-1630, with 4-5 weekend shifts in a 6-week period. Holiday coverage required for 3 out of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).

Job Description

The Registered Nurse, Case Manager will work within an Acute Care setting. The role requires a focus on patient care coordination, discharge planning, and utilization review. The nurse will interact with patients across various age groups including neonates, adolescents, adults, and geriatrics, managing a case load of up to 25 patients.

Responsibilities

• Conduct assessments and develop treatment plans for patients.
• Coordinate discharge planning and facilitate family meetings.
• Ensure compliance with CMS, Joint Commission, and other regulatory bodies.
• Manage prior authorizations and arrange DME and transport services.
• Collaborate with an interdisciplinary team including physical therapy and social services.

Required Experience

• Minimum of 2 years of acute care experience.
• Prior experience in case management and discharge planning is essential.
• Must possess a California state nursing license (pending license accepted with the requirement of being licensed upon start).

Certifications/Licensure

• California RN License required.
