Schedule/Shift

Required to work every other weekend, Day shift from 08:00 to 16:30.

Job Description

The Registered Nurse in Case Management will work in a dynamic acute care environment and will be responsible for managing patient care, coordinating treatment plans, and ensuring compliance with regulations. Utilizing Epic for documentation, the nurse will collaborate with interdisciplinary teams while advocating for patient needs.

Responsibilities

• Coordinate patient care and discharge planning.
• Conduct patient assessments and establish care plans.
• Collaborate with healthcare teams to facilitate optimal patient outcomes.
• Ensure compliance with local, state, and federal regulations.
• Provide education on benefits eligibility and appeals processes.

Required Experience

• Minimum of 2 years of acute care experience.
• Experience in case management is required.

Certifications/Licensure

• Active RN license in the state of California or a compact license.
