ARMStaffing is looking for a Registered Nurse, Case Manager (Day Shift) in Apple Valley, California.

Assignment Duration

This is a 13-week contract position.

Schedule/Shift

Shifts will be from 08:00 to 16:30, including 4-5 weekend shifts in a 6-week period. Must be willing to work every other weekend and commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).

Job Description

As a Registered Nurse Case Manager, you will be responsible for overseeing discharge planning and care coordination for patients in an acute care setting. This includes participating in family meetings, developing treatment plans, facilitating prior authorizations, and ensuring compliance with care guidelines. You will work closely with interdisciplinary teams to provide comprehensive care for patients of all ages, from neonates to geriatrics.

Responsibilities

• Conduct assessments and evaluations for care coordination.
• Plan and implement discharge plans in collaboration with healthcare teams.
• Maintain detailed documentation and ensure compliance with regulatory standards.
• Coordinate with Physical Therapy, Social Services, and other support services.
• Participate in treatment plan development and family meetings.

Required Experience

• Minimum of 2 years of experience in Acute Care Case Management or Discharge Planning.
• Hospital experience is required; behavioral health experience does not apply.

Certifications/Licensure

• California State RN License required; pending licenses accepted but must be licensed upon start date.
