This is a 13-week contract position.

Schedule/Shift

8:00 AM to 5:00 PM, with weekend rotation required.

Job Description

This position involves implementing a full-time professional nursing service to patients in their homes, under the supervision of the Clinical Services Supervisor, according to agency standards and policies. The RN will initiate and provide skilled nursing care and health counseling aimed at treating patients' medical conditions and promoting rehabilitation.

Responsibilities

• Make initial patient visits to assess physical and emotional status, needs, and resources.
• Prepare and implement a nursing plan of care aligned with the physician's treatment plan.
• Provide patient education and support based on the Care Plan/Standard of Care.

Required Experience

• 2+ years of Hospice experience is required.

Certifications/Licensure

• BLS (AHA) certification is required.
• Valid Michigan state nursing license is required.
