Required to work every other weekend. Day shift from 8:00 AM to 4:30 PM.

Job Description

The Registered Nurse in Case Management will provide comprehensive care coordination for patients with a focus on acute care. This role involves discharge planning, managing patient appeals and denials, and ensuring benefits eligibility. The nurse will also provide education concerning admission criteria and will coordinate care with various healthcare professionals.

Responsibilities

• Manage a caseload of up to 25 patients.
• Facilitate effective discharge planning and care coordination.
• Conduct needs assessments and order necessary durable medical equipment.
• Ensure compliance with CMS guidelines and Joint Commission standards.
• Work collaboratively with interdisciplinary teams, including social services.
• Assist with prior authorizations and protective services as needed.

Required Experience

To be considered for this position, candidates must have at least 2 years of acute care experience, including experience with HMOs and IPAs. Additionally, experience with advance directives is required. Candidates must possess a valid compact RN license at the time of submission.

Certifications/Licensure

Valid compact RN license is required.
