Job Description

The Registered Nurse, Case Manager will play a critical role in patient care within an acute care setting. Responsibilities include care coordination and discharge planning for patients across various age groups, including adolescents, adults, and geriatrics. The candidate will utilize the Epic charting system and will be expected to manage a patient ratio of 1:20.

Responsibilities

• Coordinate patient care through effective discharge planning.
• Conduct family meetings to discuss treatment plans and progress.
• Utilize InterQual criteria for patient assessments.
• Arrange durable medical equipment (DME), skilled nursing facility (SNF) admissions, and ambulance transport as required.
• Provide knowledge and support for patients experiencing substance abuse, homelessness, and mental health crises.

Required Experience

• Minimum 2 years of acute care hospital case management experience.
• Previous experience in care coordination and discharge planning is essential.
• Experience with assessing patients and developing individualized treatment plans.

Certifications/Licensure

• Valid CA state license required; pending licenses accepted if the candidate is licensed upon start date.
