ARMStaffing is looking for a Medical Social Worker, Acute Care (Day Shift) in Burbank, California.

Assignment Duration

This is a 13-week contract position.

Schedule/Shift

This position requires working typically on one full weekend every four weeks, and you must be available to work three of the five holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day. The standard shift is from 8:00 AM to 4:30 PM, Monday through Friday.

Job Description

The Medical Social Worker will perform both social work and discharge planning roles within an acute care facility. You will participate in care conference activities and develop plans of care that meet patient and caregiver needs. Additionally, the position involves organizing family meetings, assessing needs for durable medical equipment (DME), and addressing various social and health issues, including substance abuse, mental health crises, and homelessness.

Responsibilities

• Conduct needs assessments and develop individualized plans of care.
• Facilitate discharge planning and ensure continuity of care.
• Organize and participate in family meetings to discuss treatment plans.
• Collaborate with interdisciplinary teams and participate in care conferences.
• Address psychosocial issues related to medical conditions and care transitions.

Required Experience

Minimum of 2 years of acute medical social work experience is required, along with experience performing discharge planning.

Certifications/Licensure

Must hold a Master's in Social Work (MSW) and possess a Licensed Clinical Social Worker (LCSW) certification or be in the process of obtaining it.
