Job Description

The Physical Therapist will evaluate assigned patients and develop, implement, and modify individualized treatment plans according to their needs, goals, and physician directives. The role involves delegating and directing all therapy services for assigned patients to ensure effective community reintegration.

Responsibilities

• Conduct comprehensive evaluations of patients and create appropriate treatment plans.
• Document patient care in an accurate and timely manner, including goal writing and functional assessments.
• Ensure care is compliant with physician orders and insurance approval.
• Serve as an educator for patients, families, students, and team members.
• Participate in the development of therapy services and program delivery.
• Engage in professional development and continuing education activities to enhance clinical skills.
• Provide supervision and support for students participating in the student program.
• Adhere to professional standards and the organizational Code of Conduct, prioritizing safety in care delivery.

Required Experience

• Bachelor’s or Doctorate in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• CPR Certification.

Certifications/Licensure

• Active Physical Therapist license in good standing.
• Completion of all state-mandated continuing education requirements.
