Monday to Friday, 5 × 8 hour shifts, 40 hours per week.

Job Description

The Physical Therapist will work in a home health setting, providing therapy services to patients in Ellis County and potentially within a 60-mile radius. This position involves an average of 4-6 patient visits per day, depending on the type of therapy session. The therapist will be required to perform evaluations, develop treatment plans, and complete necessary documentation using Homecare Homebase. Previous experience with OASIS documentation is essential.

Responsibilities

• Conduct initial assessments and develop treatment plans for patients.
• Utilize OASIS for documentation of patient evaluations and progress.
• Perform routine visits and follow up with patients to ensure effective care.
• Participate in weekly case conferences to discuss patient progress.

Required Experience

• Home health and OASIS experience required.
• Proficiency in Homecare Homebase documentation preferred.

Certifications/Licensure

• Valid Physical Therapist license is required.
