Day shift, 5x8 hours (08:00 - 16:30). Required to work every other weekend.

Job Description

Join our team as a Case Manager in Acute Care, where you will play a crucial role in patient care and management. Your responsibilities will include coordinating patient care, overseeing discharge planning, and ensuring compliance with CMS guidelines. This position requires a strong background in case management within an acute hospital setting.

Responsibilities

• Coordinate and manage patient care services.
• Facilitate discharge planning and ensure continuity of care.
• Conduct patient assessments and develop care plans.
• Manage appeals, denials, and benefits eligibility.
• Collaborate with interdisciplinary teams to provide comprehensive care.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute care experience required.
• Experience with HMOs, IPAs, and advance directives preferred.
• Current valid state license is required at the time of submission.
• Basic knowledge of HIPAA guidelines and CMS regulations.
• Must have experience with Epic charting system.
