Case Manager - Acute Care

Job Description

This is a 13-week contract position for a Case Manager specializing in Acute Care settings. The role requires a dedicated professional with experience in patient care coordination and case management.

Responsibilities

• Coordinate patient care and ensure the implementation of discharge planning processes.
• Conduct needs assessments and manage care coordination for diverse patient age groups, including pediatrics, adults, and geriatrics.
• Manage a caseload of up to 25 patients, ensuring efficient utilization of resources.
• Navigate and comply with CMS guidelines, HIPAA regulations, and insurance requirements.
• Perform admissions, appeals, denials, and prior authorizations as needed.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute care case management experience required.
• Experience with HMOs and IPAs.
• Current State Nursing License (Compact license preferred).
• Familiarity with Epic charting system is essential.
• Strong understanding of advance directive and protective services processes.

Schedule

Day shift, 8-hour schedules from 08:00 to 16:30, with a requirement to work every other weekend.
