Day Shift: 5x8-Hour (08:00 - 16:30), with every other weekend required.

Job Description

We are seeking an experienced Case Manager with acute care experience to join our team. This role is focused on care coordination, discharge planning, and managing case loads effectively to meet patient needs.

Responsibilities

• Coordinate care for patients, ensuring compliance with CMS guidelines.
• Manage admission criteria and discharge planning efficiently.
• Conduct needs assessments and facilitate prior authorizations.
• Work collaboratively with interdisciplinary teams and social services.
• Handle appeals, denials, and ensure benefits eligibility for patients.

Required Experience / Certifications / Licensure

• A minimum of 2 years of acute care case management experience required.
• Experience with HMOs and IPAs strongly preferred.
• Competence in using Epic charting system is mandatory.
• Current relevant licensure is required.
