Day shift: 5x8 hours (08:00 - 16:30). Required to work every other weekend.

Job Description

We are seeking an experienced Case Manager for an acute care setting. The candidate will be responsible for coordinating patient care, managing case loads, and ensuring compliance with healthcare regulations. Strong knowledge of admissions, discharge planning, and benefits eligibility is essential. The ideal candidate should have experience in handling appeals and denials, as well as proficiency in the Epic charting system.

Responsibilities

• Coordinate patient care and discharge planning.
• Manage a case load of up to 25 patients.
• Ensure compliance with CMS and HIPAA guidelines.
• Conduct needs assessments and facilitate prior authorizations.
• Actively participate in interdisciplinary team meetings.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute care case management experience.
• Valid RN license required (Compact license must be in hand at submission).
• Familiarity with HMOs, IPAs, and advance directive processes.
• Experience with protective services is a plus.
