LPN - Full Time Nights

This position requires full-time commitment, working 5x8 hours per week, including every other weekend and holidays, with a flexible end date.

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing direct and indirect skilled nursing care for assigned residents, demonstrating the necessary knowledge and skills to care for adult and geriatric patients. The LPN will work under the supervision of a Registered Nurse and maintain collaborative relationships with nursing staff, physicians, and ancillary staff.

Responsibilities

• Collect ongoing data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and Interdisciplinary Team.
• Contribute to the creation of culturally relevant and age-appropriate care plans.
• Recommend revisions to care plans based on evaluation of resident outcomes.
• Provide leadership under the direction of the RN, ensuring customer satisfaction and a safe environment.
• Participate in the mentoring and orientation of new staff.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• Basic Life Support (BLS) certification through AHA or RQI
• Minimum of 1 year of experience serving in an LPN capacity in a Long-Term Care (LTC) facility
