Job Description

The Licensed Practical Nurse (LPN) will demonstrate the knowledge and skills necessary to provide age-appropriate care for patients on the assigned unit. The LPN is responsible for assessing data regarding the patient's status and interpreting the information needed to identify each patient's requirements related to their specific needs, particularly in adult and geriatric care.

Responsibilities

• Collect ongoing data relevant to resident's physical and psychosocial condition and communicate findings to the Registered Nurse (RN) and interdisciplinary team.
• Contribute to developing culturally relevant care plans reflective of nursing diagnoses and interventions.
• Recommend revisions to care plans based on evaluation of resident outcomes and initiate referrals as needed.
• Collaborate with residents, families, and the interdisciplinary team to ensure comprehensive care and promote a healthy, safe environment.
• Maintain knowledge and competency in nursing practices through continuing education and mentorship of staff.
• Provide leadership in the professional practice setting under the direction of the RN.
• Manage tasks that involve potential exposure to blood, body fluids, or tissues.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure.
• BLS (AHA or RQI certification required).
• Minimum of one year experience as an LPN, preferably in a long-term care (LTC) facility.
