Part-Time LPN - Evenings

Assignment Duration

This is a 13-week contract position.

Schedule

Part-time evenings, specifically 2 shifts of 8 hours one week and 3 shifts of 8 hours the following week, with every other weekend required. Availability during the holiday season and January is mandatory.

Job Description

The LPN will provide essential nursing care tailored to the needs of adult and geriatric patients. This role involves close collaboration with the interdisciplinary team under the supervision of a Registered Nurse.

Responsibilities

• Collect ongoing data related to resident's physical and psychosocial condition and communicate findings to the RN and interdisciplinary team.
• Contribute to creating patient care plans that are age-appropriate and culturally relevant, involving residents and their families in the process.
• Recommend adjustments to care plans based on patient outcomes and identify referrals necessary for comprehensive care.
• Maintain knowledge and competency in nursing practices, participating in staff mentoring and orientation.
• Operate within established safety and cost protocols while planning and delivering care.

Required Experience / Certifications / Licensure

• Current New York LPN Licensure.
• Basic Life Support (BLS) certification through AHA or RQI.
• Minimum of 1 year of experience in a long-term care (LTC) facility.
