Full-time, 5x8 shifts. Day, Evening, Night, and Weekend rotations required.

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents. The LPN must demonstrate the knowledge and skills necessary to provide age-appropriate care and possess the ability to assess and interpret information relevant to each patient's requirements relative to his or her age-specific needs. This position involves collaborating with the healthcare team and maintaining a safe, healthy environment for residents, specifically focusing on adult and geriatric patient groups.

Responsibilities

• Collect and communicate data regarding residents' physical and psychosocial conditions to the RN and interdisciplinary team.
• Contribute to the development of culturally relevant care plans reflecting nursing diagnosis and interventions.
• Recommend revisions to care plans based on evaluation of resident outcomes and ensure the need for referrals is met.
• Engage residents and families in the care process while promoting customer satisfaction.
• Maintain ongoing knowledge and competency in nursing practice and participate in mentoring or orientation of staff.

Required Experience / Certifications / Licensure

• New York LPN Licensure
• BLS (AHA or RQI only)
• 1 year of experience in a Long-Term Care (LTC) facility
