Full-Time Evenings, Every Other Weekend (EOW). Contract starts on Tuesday or Thursday.

Job Description

The Licensed Practical Nurse (LPN) will demonstrate the knowledge and skills necessary to provide appropriate care to residents, particularly within the adult and geriatric patient groups. Under the supervision of a Registered Nurse, the LPN will maintain a collaborative relationship with nursing staff, physicians, and ancillary personnel to ensure high-quality patient care.

Responsibilities

• Collect and assess ongoing data relevant to residents' physical and psychosocial condition, communicating findings to the RN and interdisciplinary team.
• Contribute to the development of a culturally relevant plan of care that reflects nursing diagnoses and interventions.
• Review and adjust the plan of care based on resident outcomes and initiate referrals as necessary.
• Maintain clinical knowledge and competencies; participate in mentorship and orientation of new staff.
• Provide leadership in practice settings and demonstrate knowledge of safe nursing practices.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• Basic Life Support (BLS) from AHA or RQI
• Minimum of 1 year experience as an LPN in a Long-Term Care (LTC) facility
