Licensed Practical Nurse (LPN) - Long-Term Care

Job Description

The Licensed Practical Nurse (LPN) will provide comprehensive nursing care to geriatric residents in a long-term care facility. The LPN is responsible for demonstrating the knowledge and skills necessary to care for patients of various ages, with a focus on adult and geriatric populations. The nurse will work collaboratively with the healthcare team to ensure the delivery of high-quality care in accordance with established policies and procedures.

Responsibilities

• Collect data relevant to residents' physical and psychosocial conditions on an ongoing basis and communicate findings to the Registered Nurse and interdisciplinary team.
• Contribute to the development of an age-appropriate and culturally relevant care plan, involving the resident/family and healthcare team.
• Recommend revisions to the care plan based on resident outcomes and indicate the need for referrals to address all aspects of care.
• Participate in maintaining current knowledge and competency in nursing practice through continuing education and clinics.
• Provide leadership within the nursing practice setting under the direction of the Registered Nurse.
• Engage in tasks with potential exposure to blood-borne pathogens, ensuring safety and compliance with health regulations.

Required Experience / Certifications / Licensure

• Valid New York LPN Licensure
• Current BLS certification
• Minimum of 1 year of experience as a Licensed Practical Nurse in a long-term care setting
• Able to work 24 hours per 4 weeks, including 2 weekend shifts

Schedule

This position requires flexibility to work shifts starting on either Tuesday or Thursday, running through the holiday season.
