Licensed Practical Nurse (LPN) - Full Time

Schedule

All contracts must run through the holiday season with an end date that is flexible. This position requires a full-time schedule with day/night variable shifts and a commitment to work every weekend (40 hours per week).

Job Description

The Licensed Practical Nurse (LPN) plays a critical role in providing care to patients in a long-term care setting. The LPN is responsible for demonstrating the knowledge and skills necessary to care for patients and ensuring the implementation of individualized care plans suitable for adults and geriatric residents. The position requires collaboration with other healthcare professionals to ensure optimal patient outcomes.

Responsibilities

• Collect ongoing data related to resident's physical and psychosocial conditions and communicate findings with the RN and interdisciplinary team members.
• Contribute to the development of culturally relevant care plans based on nursing diagnoses and interventions.
• Recommend revisions to care plans based on evaluations of resident outcomes and indicate the need for referrals to address all aspects of care.
• Participate in mentoring and orientation of new staff while maintaining a high level of clinical expertise.
• Provide leadership in the practice setting under the direction of the RN and ensure compliance with safety and cost-related factors in care delivery.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure and BLS certification.
• A minimum of 1 year of experience as an LPN in a Long-Term Care facility.
