Part-Time Licensed Practical Nurse (LPN)

Schedule

Part-time Day and Evening shifts, scheduled as needed by facility (generally 2x8s/3x8s) plus weekend and holiday shifts required.

Job Description

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on their assigned unit. The individual must possess the ability to assess data reflective of the patient's status and interpret the information needed to identify each patient's requirements relative to their age-specific needs. The LPN shall provide direct and indirect skilled nursing care for assigned residents, following established policies and procedures under the supervision of a Registered Nurse.

Responsibilities

• Collect data relevant to residents' physical and psychosocial conditions on an ongoing basis, and communicate findings to the RN and other members of the Interdisciplinary Team.
• Contribute to the creation of a developmentally appropriate and culturally relevant plan of resident care that reflects nursing diagnosis and interventions while involving the resident/family and healthcare team in the process.
• Recommend revisions to the plan of care based on evaluation of resident outcomes and indicate the need for referrals to ensure comprehensive care.
• Maintain current knowledge and competency in nursing practice and participate in the mentoring/orientation of staff.
• Provide leadership in the professional practice setting under the direction of the RN while considering safety, cost, and impact in planning care delivery.

Required Experience / Certifications / Licensure

• Current New York LPN Licensure.
• BLS certification through AHA or RQI.
• Experience serving as an LPN in a Long-Term Care (LTC) facility (minimum of 1 year).
