LPN Full Time (Nights, 5x8)

Job Description

The Licensed Practical Nurse (LPN) will provide both direct and indirect skilled nursing care to assigned residents while following defined policies and procedures under the supervision of a Registered Nurse. This position requires the ability to assess patient's needs, demonstrate knowledge of age-appropriate care for adult and geriatric populations, and work collaboratively with other healthcare team members.

Responsibilities

• Collect ongoing data relevant to residents' physical and psychosocial condition and communicate findings to the Registered Nurse and interdisciplinary team members.
• Create developmentally appropriate and culturally relevant care plans in collaboration with the resident, family, and healthcare team.
• Evaluate resident outcomes and recommend revisions to the care plan as needed.
• Participate in the mentoring and orientation of new staff while ensuring a safe and healthy environment.
• Maintain current knowledge and competency in nursing practice and demonstrate leadership in the professional setting.

Required Experience / Certifications / Licensure

• Valid NY LPN Licensure
• BLS certification (through AHA or RQI)
• Minimum of 1 year of experience as an LPN, preferably in a long-term care facility

Schedule

This is a full-time position (40 hours per week) working nights, with a schedule of 5x8 hours. Weekend and holiday shifts are required.
