Licensed Practical Nurse (LPN)

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents within a long-term care facility. The LPN will work under the supervision of a Registered Nurse and must demonstrate the knowledge and skills necessary to care for adult and geriatric patient groups, reflecting the principles of growth and development throughout the lifespan.

Responsibilities

• Collect and assess data relevant to the resident's physical and psychosocial condition, communicating findings to the Registered Nurse and interdisciplinary team.
• Develop and implement age-appropriate and culturally relevant care plans reflecting nursing diagnoses and interventions.
• Evaluate resident outcomes and recommend revisions to the care plan as necessary.
• Collaborate with residents, families, and healthcare team members to ensure comprehensive care and customer satisfaction.
• Maintain current knowledge and competency in nursing practices; participate in mentoring and orientation of staff.
• Provide leadership in the professional practice setting under the direction of the Registered Nurse.
• Adhere to safety protocols regarding potential exposures to blood and body fluids.

Required Experience & Certifications

• Current New York State LPN Licensure
• BLS Certification
• Minimum of 1 year experience as an LPN in a long-term care facility
• Availability to work a minimum of 24 hours per 4-week schedule, including 2 weekend shifts

Schedule

Candidate must be available to start on a Tuesday or Thursday, full shift.
