Licensed Practical Nurse (LPN) - Full Time Evenings

This is a full-time position requiring evening shifts of 5x8 hours per week, including every other weekend and holiday.

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing direct and indirect skilled nursing care for assigned residents. The LPN will work under the supervision of a Registered Nurse and must demonstrate knowledge and skills necessary to care for adult and geriatric patients. The role includes assessment, planning, implementation, and evaluation of individualized patient care plans.

Responsibilities

• Collect and analyze data relevant to the resident's physical and psychosocial condition and communicate findings to the RN and interdisciplinary team.
• Collaborate with residents, families, and healthcare team members to create and adjust age-appropriate and culturally relevant care plans.
• Engage in mentoring and orientation of new staff, promoting a safe and efficient nursing practice environment.
• Maintain competencies through continuing education and inservice programs, ensuring leadership in professional practice settings.
• Adhere to infection control protocols and safety measures associated with potential exposure to blood borne pathogens.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS (AHA or RQI) certification required
• Minimum of 1 year of experience as an LPN in a long-term care facility
