Outpatient Physical Therapist

Job Description

We are seeking a dedicated Outpatient Physical Therapist to join our team for a 6-month contract. The ideal candidate will be responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans in accordance with patient needs, goals, and physician direction. This role demands a focus on delivering superior quality care while ensuring patient and employee safety.

Responsibilities

• Perform comprehensive evaluations for all assigned patients.
• Develop individualized treatment plans and set measurable goals.
• Document patient care in a clear, concise, and timely manner.
• Ensure all care is provided under signed, valid physician orders and in compliance with related guidelines.
• Participate in a student program as assigned, providing supervision and mentorship.
• Engage in ongoing professional development and pursue continuing education opportunities.
• Maintain compliance with discipline-specific State Practice Act and continuing education requirements.
• Demonstrate commitment to the organization’s performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 6 months to 1+ years of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure as a Physical Therapist.

Schedule

This position involves a 5x8.0 schedule with shifts that may be subject to change.
