Licensed Practical Nurse (LPN) - Full Time Nights

Full Time Nights, 5x8 hours a week. Weekends and Holidays required.

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care to residents, demonstrating the knowledge and skills necessary to care for patients of various ages, primarily adults and geriatrics. The LPN will work under the supervision of a Registered Nurse and must maintain collaborative relationships with nursing staff, physicians, and ancillary personnel. Continuous education and clinical expertise are essential components of this role, ensuring the delivery of safe and effective patient care.

Responsibilities

• Conduct ongoing assessments of residents' physical and psychosocial conditions, communicating findings to nursing staff and interdisciplinary team members.
• Develop and implement age-appropriate and culturally relevant care plans involving residents and families.
• Review and revise care plans based on evaluations of resident outcomes, ensuring all aspects of care are addressed effectively.
• Participate in mentoring and orienting new staff members, contributing to a culture of safety and learning.
• Provide leadership within the professional practice setting by demonstrating knowledge and competence in nursing.
• Maintain awareness of safety practices and costs while planning and delivering nursing care.
• Handle occupational hazards, including potential exposure to blood-borne pathogens.

Required Experience / Certifications / Licensure

• Current New York State LPN Licensure.
• BLS certification from the American Heart Association or RQI.
• Minimum of 1 year of experience as an LPN, preferably in a long-term care facility.
