LPN Per Diem

Job Description

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs. The LPN shall provide care primarily for adult and geriatric patient groups.

Responsibilities

• Collect data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and other members of the Interdisciplinary Team.
• Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnoses and interventions.
• Recommend revisions to the plan of care based on evaluation of resident outcomes and collaborate with the healthcare team for comprehensive care.
• Maintain current knowledge and competency in nursing practice, participating in mentoring and orientation of staff.
• Provide leadership in the professional practice setting under the direction of the RN.

Required Experience / Certifications / Licensure

• New York LPN Licensure (required)
• BLS certification through AHA/RQI (required)
• Minimum 1 year of experience as an LPN, preferably in a long-term care facility (required)

Schedule

MUST be able to start full-shift orientation on a Tuesday or Thursday. EOW and Holiday shifts are required.
