Job Description

The Licensed Practical Nurse (LPN) will demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, providing care primarily to adult and geriatric patient groups.

Responsibilities

• Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicate findings to the RN and other members of the Interdisciplinary Team.
• Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions.
• Recommend revisions to the plan of care after evaluating resident outcomes and indicating the need for referrals.
• Collaborate with residents/families and members of the Interdisciplinary Team to ensure customer satisfaction while promoting a healthy, safe environment.
• Maintain current knowledge and competency in nursing practice and participate in mentoring/orientation of staff.
• Provide leadership in the professional practice setting under the direction of the RN.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS Certification
• At least 1 year of experience serving in an LPN capacity at a long-term care facility
