This is a 13-week contract position.
Schedule
Candidate must be available to start on a Tuesday or Thursday, full shift. Minimum shift requirement is 24 hours per 4 weeks with 2 weekend shifts required.
Job Description
The Licensed Practical Nurse (LPN) will demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The LPN must possess the ability to assess and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs and to provide the care required by adult and geriatric patient groups. This role involves providing direct and indirect skilled nursing care under the supervision of a Registered Nurse (RN), while maintaining a collaborative relationship with other nursing staff, physicians, and ancillary staff.
Responsibilities
- Collect data relevant to resident's physical and psychosocial condition on an ongoing basis and communicate findings to RN and other members of the Interdisciplinary Team.
- Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of care reflecting nursing diagnosis and interventions.
- Recommend revisions to the plan of care after evaluating resident outcomes, indicating the need for referrals as necessary.
- Collaborate with residents/families and members of the Interdisciplinary Team to ensure customer satisfaction.
- Maintain current knowledge and competency in Nursing Practice and participate in the mentoring/orientation of staff under the direction of the RN.
Required Experience / Certifications / Licensure
- Current NY LPN Licensure
- Basic Life Support (BLS) certification
- Minimum 1 year of experience serving in an LPN capacity at a Long Term Care (LTC) facility
