Part-Time LPN (24 hours) - Evenings

Job Description

The Licensed Practical Nurse (LPN) will perform direct and indirect resident care under the direction of licensed nursing personnel within the scope of practice defined by certification. This role will include assessing and providing age-appropriate care for adult and geriatric patients, ensuring that their needs are met as outlined in the individual plan of care.

Responsibilities

• Assist residents with activities of daily living (ADLs) as outlined in the plan of care, including hygiene, grooming, dressing, and toileting needs.
• Maintain resident safety and cleanliness in accordance with policies and procedures.
• Meet resident nutritional needs as listed in the plan of care, including meal delivery and feeding assistance.
• Provide rehabilitative care as directed, including range of motion exercises and tranfers.
• Report any changes in resident condition to the Charge Nurse in a timely manner.
• Adhere to standards regarding occupational exposure to bloodborne pathogens.

Schedule

Part Time Evenings required (2 shifts of 8 hours, or 3 shifts of 8 hours a week). Must be available for full shift orientation on a Tuesday or Thursday.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure.
• BLS Certification (AHA or RQI only).
• Minimum of 1 year experience as an LPN, preferably in a long-term care (LTC) or comparable capacity.
• High School Diploma or GED required.
