Part Time Evenings (2x8s/3x8s), must be available for full shift orientation on a Tuesday or Thursday.

Job Description

The Licensed Practical Nurse (LPN) will perform direct and indirect resident care under the guidance of licensed nursing personnel. This role involves meeting the diverse needs of patients, adhering to established policies and procedures, and documenting care as per protocol.

Responsibilities

• Ensure residents' activities of daily living (ADLs) are met, including personal hygiene, grooming, dressing, and toileting needs.
• Maintain residents' safety and comfort, promptly addressing call lights and alarms while ensuring a clean environment.
• Assist with fulfilling residents' nutritional needs, including meal delivery and adaptive feeding techniques.
• Support residents’ rehabilitative needs, including ambulation, transfers, and use of assistive devices.
• Report residents' rights, refusals, concerns, and any changes in their physical, emotional, or psychosocial status to the Charge Nurse.

Required Experience / Certifications / Licensure

• New York LPN Licensure
• BLS certification (AHA or RQI only)
• At least one year of experience as an LPN or in a long-term care setting
