Mid Shift: 5x8 hours (15:00 - 23:00), including every other weekend and holiday availability.

Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) with experience in a nursing facility/nursing home setting. The ideal candidate will possess strong patient care skills and the ability to manage various nursing responsibilities, ensuring quality care for our residents.

Responsibilities

• Provide direct patient care and assist with daily living activities.
• Monitor and document patient vitals and health status using EPIC charting.
• Utilize behavior management techniques for patients with degenerative disorders such as Alzheimer’s, ALS, and Parkinson’s.
• Administer medications and treatments as prescribed.
• Communicate changes in patient status to RN/MD.
• Ensure compliance with all nursing protocols and procedures.

Required Experience / Certifications / Licensure

• Active New York State LPN license.
• Minimum of 1 year of nursing experience in a nursing facility or nursing home.
• BLS certification required.
• Experience with EPIC charting system is mandatory.
• Familiarity with obtaining 12 lead EKG/ECG.
