Physical Therapist - Outpatient

Job Description

The physical therapist will evaluate all assigned patients and develop, implement, and modify treatment plans in accordance with patients' needs, goals, and physician direction. The therapist is responsible for delegating and directing all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that facilitate an effective return to community activities. This role also includes serving as an educator for patients, families, students, and coworkers, while upholding the organization's Mission Statement and Core Values.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document care provided in compliance with state, federal, and organizational guidelines.
• Ensure patient care is delivered under signed, valid physician orders and within insurance approval.
• Participate in student programs, providing supervision and evaluation.
• Create and actively pursue a professional development plan.
• Adhere to discipline-specific State Practice Act and maintain professional licensure.
• Contribute to organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 6 months to 1+ year of outpatient clinical experience.
• Current CPR certification.

Schedule

This position is a 5x8.0 shift based on a contract of 13 weeks.
