Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. Responsibilities include the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective return to community activities. This role also involves serving as an educator for patients, families, students, coworkers, and self while performing other professional duties associated with patient care and the development of discipline-specific therapy services.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document care in a clear, concise, accurate, and timely manner.
• Ensure care is provided under signed, valid physician orders and within insurance approval.
• Participate in student program as assigned, providing a positive learning environment.
• Create an individualized professional development plan with clinical management.
• Adhere to discipline-specific State Practice Act and other regulatory agencies.
• Demonstrate awareness of organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1+ year of outpatient clinical experience.
• CPR Certification required.
• Maintains professional licensure in good standing and completes all state-mandated continuing education requirements.

Schedule

This position follows a 5x8.0 schedule with evaluations scheduled for 60 minutes followed by follow-ups every 30 minutes.
