RN Case Manager - Emergency Department and Medical Inpatient Coverage

Assignment Duration

This is a 13-week contract position.

Schedule

5 x 8-hour shifts (08:30 - 17:00), every 4th weekend, with potential for up to every other weekend. One holiday per year as staff.

Job Description

The RN Case Manager will focus on Emergency Department (ED) and medical inpatient coverage. The ideal candidate is expected to be able to hit the ground running with approximately 3 days of orientation and work collaboratively with case managers and social workers.

Responsibilities

• Provide case management services for patients in a medical inpatient and emergency department setting.
• Coordinate discharge planning, ensuring smooth transitions and continuity of care.
• Work with a patient ratio of ideally 1 unit (20 beds) and in partnership with other staff.

Required Experience / Certifications / Licensure

• Minimum of 2 years of hospital inpatient med-surg or critical care experience with a focus on discharge planning.
• Valid RN license in the state of Maryland or Compact license required at the time of submission.
• BLS certification required.

Patient Care Details

• Patient Age Groups: Adults, Geriatrics
• Daily Census: 40 patients
• Common Patient Diagnoses: General Medical conditions including diabetes, heart failure, COPD, and others.
