Day Shift: 5x8 Hour Shifts (08:30 - 17:00), every 4th weekend required.

Job Description

The RN Case Manager will focus on Emergency Department and medical inpatient coverage. The candidate will be responsible for managing patient care transitions, ensuring appropriate discharge planning, and collaborating with interdisciplinary teams.

Responsibilities

• Provide case management services for patients within assigned inpatient units.
• Collaborate with social workers and other staff to support patient care.
• Ensure effective discharge planning and coordination of services.
• Manage a patient caseload efficiently, ideally focusing on 1 unit (20 beds).
• Participate in team meetings and provide updates on patient progress and planning.

Required Experience / Certifications / Licensure

• Minimum of 2 years experience in hospital inpatient medical-surgical or critical care discharge planning.
• Active Registered Nurse (RN) license in Maryland or Compact state.
• Previous charge experience is a plus.
• Basic Life Support (BLS) certification is required.
