Day shift, Monday through Friday, 5x8 hours (08:00 - 16:30)

Job Description

The Case Manager RN will be responsible for providing and coordinating medical home care, placement, and addressing social issues to ensure safe discharges for patients. This role will involve managing an average of 15 cases daily, focusing on inpatient case management.

Responsibilities

• Coordinate and execute discharge planning and care transitions.
• Evaluate patient needs and develop appropriate care plans.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Address and manage social issues impacting patient discharges.

Required Experience / Certifications / Licensure

• Registered Nurse (RN) license in Maryland or compact state.
• Minimum of 2 years of experience in a hospital or acute care setting.
• Previous inpatient case management experience preferred.
• BLS certification preferred.
• Experience with EPIC charting is advantageous.
