Day Shift: 5x8 hours, Monday through Friday from 08:00 AM to 04:30 PM

Job Description

The Case Manager is responsible for coordinating inpatient medical home care, placements, and addressing social issues to ensure safe discharges. The role focuses on providing wrap-around services to patients and involves working with an average daily census of 15 cases.

Responsibilities

• Manage and coordinate medical home care and placement issues for patients.
• Address social issues and provide wrap-around services to ensure safe discharges.
• Conduct case management for inpatient settings while utilizing EPIC charting.
• Collaborate with interdisciplinary teams to meet patient needs.
• Float between facilities as required, operating within a 10-mile radius.

Required Experience / Certifications / Licensure

• Must possess an active Maryland LCSW or LCSW-C license for Social Workers or a Maryland state nursing license for Registered Nurses.
• 3 years of experience in inpatient case management is required; first-time travelers with relevant experience will be considered.
• EPIC charting experience is preferred.
• BLS certification is preferred.
