Licensed Practical Nurse (LPN) - Full Time Nights

Schedule

This is a full-time position working nights, with a schedule of 5x8 per week. Weekend and holiday shifts are required.

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care to residents, demonstrating knowledge and skills tailored to the developmental needs of patients across the life span. This position requires maintaining a collaborative relationship with nursing staff, physicians, and ancillary staff while adhering to policies and procedures under the supervision of a Registered Nurse.

Responsibilities

• Collect and analyze data regarding residents' physical and psychosocial conditions, communicating findings to the RN and interdisciplinary team.
• Contribute to the development of culturally relevant care plans that reflect nursing diagnoses and interventions to achieve desired outcomes.
• Evaluate resident outcomes and recommend revisions to care plans, ensuring comprehensive care and collaboration with residents and families.
• Participate in maintaining clinical expertise through ongoing education and mentorship of staff.
• Provide leadership in the professional practice setting under the guidance of the RN.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification (AHA or RQI)
• Minimum of 1 year experience as an LPN, preferably in a long-term care facility
