This is a 13-week contract position.

Schedule

Day shift, 5x8 hours, Monday through Friday, typically from 08:00 AM to 04:30 PM.

Job Description

The Case Manager RN will oversee guardianships, complex medical cases, placements, and social situations. This role requires strong coordination of medical home care and discharge planning services to ensure safe patient transitions.

Responsibilities

• Provide comprehensive case management and discharge planning.
• Conduct assessments and coordinate care for patients.
• Manage and oversee complex medical cases and social issues.
• Ensure proper placements and medical home care interventions.

Required Experience / Certifications / Licensure

• Active RN license in Maryland required; LCSW with 3 years of experience will be accepted.
• Experience with Epic charting is mandatory.
• Inpatient hospital/acute care experience is essential.
• BLS certification preferred.
