Case Manager RN - Days

This is a 13-week contract position.

Schedule

5x8 Days, Monday through Friday, from 08:00 or 08:30 to 16:00 or 16:30, depending on needs.

Job Description

The Case Manager RN oversees Guardianships, complex medical cases, and social situations, ensuring safe discharges through effective care coordination and planning.

Responsibilities

• Coordinate medical home care and placements for patients.
• Manage social situations affecting patient care and discharge plans.
• Conduct assessments and SDOH screenings as part of the case management process.
• Provide support and resources for outpatient and post-acute services.

Required Experience / Certifications / Licensure

• Must have a valid RN license in Maryland.
• 3 years of inpatient hospital or acute care experience required; first-time travelers accepted.
• BLS certification preferred.
• Experience with EPIC charting preferred.
