Licensed Practical Nurse (LPN) - Full Time Evenings

Full Time Evenings, 5x8 shifts per week. Every other weekend and holiday required.

Job Description

The Licensed Practical Nurse (LPN) is responsible for delivering high-quality nursing care for residents in a long-term care facility. The LPN must demonstrate the knowledge and skills necessary to provide appropriate care, considering the age-specific needs of the adult and geriatric patient population.

Responsibilities

• Provide direct and indirect skilled nursing care under the supervision of a Registered Nurse.
• Collect and assess data relevant to the physical and psychosocial conditions of residents and communicate findings to the RN and other members of the Interdisciplinary Team.
• Contribute to the development of culturally relevant and age-appropriate care plans reflecting nursing diagnoses and interventions.
• Evaluate resident outcomes and recommend revisions to care plans as needed.
• Promote a healthy and safe environment by collaborating with residents, families, and healthcare teams.
• Participate in mentoring and orientation of new staff while maintaining current knowledge in nursing practice.

Required Experience / Certifications / Licensure

• Current New York LPN Licensure
• BLS Certification (AHA or RQI required)
• At least 1 year of experience as an LPN in a long-term care facility
