FT Days and Evenings scheduled as needed by the client (5x8 per week) + Every Other Weekend and Holiday

Job Description

The Licensed Practical Nurse (LPN) must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on their assigned unit. The individual must be able to assess data reflecting the patient's status and interpret the necessary information to identify each patient's requirements, ensuring care that is suitable for the adult and geriatric patient groups.

Responsibilities

• Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicate findings to the RN and other members of the Interdisciplinary Team.
• Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care, reflecting nursing diagnosis and interventions to achieve desired resident outcomes.
• Recommend revisions to the plan of care after evaluating resident outcomes, indicating the need for referrals as necessary.
• Maintain current knowledge and competency in nursing practice, participating in the mentoring and orientation of staff.
• Provide leadership in the professional practice setting under the direction of the RN, ensuring a safe and effective care environment.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• Basic Life Support certification (BLS) from AHA or RQI only
• Minimum of 1 year of experience as an LPN, preferably in a long-term care facility
