Schedule

Monday-Friday, AM shift from 5 AM, 8 hours per shift, guaranteed 40 hours per week.

Job Description

We are seeking a skilled Home Health Physical, Occupational, or Speech Therapist to provide high-quality care in Fayette and Fulton counties. This role may require assistance in other territories within the facility's service area, approximately a 60-mile radius.

Responsibilities

• Conduct assessments and provide treatment plans for patients in home health settings.
• Perform an average of 4-6 visits per day for PT/OT/SLP and 6-7 visits for PTA/COTA, depending on visit type.
• Maintain productivity of 28-32 points per week for PT/OT/SLP.
• Document services using Homecare Homebase (HCHB) to ensure compliance and continuity of care.
• Participate in case conferences and regularly reassess patient progress.

Required Experience / Certifications / Licensure

• Experience in Home Health and OASIS.
• Homecare Homebase documentation experience is required; however, candidates without HCHB experience may be considered at a reduced rate during training.
• Active and valid licensure in the relevant therapy discipline (PT, OT, SLP).
