Licensed Practical Nurse (LPN) - Full Time D/E Rotation

Job Description

The Licensed Practical Nurse (LPN) will provide both direct and indirect skilled nursing care for assigned residents within their designated unit. This role requires a commitment to demonstrating the knowledge and skills necessary to meet the age-specific needs of the patient population, particularly focusing on adult and geriatric care.

Responsibilities

• Continually collect data relevant to resident's physical and psychosocial condition and communicate findings to the RN and interdisciplinary team.
• Develop and revise a culturally relevant plan of care, incorporating the resident and their family while promoting a healthy and safe environment.
• Evaluate resident outcomes and make necessary referrals to ensure comprehensive care.
• Maintain current knowledge and competency in nursing practice; participate in mentoring and orienting new staff.
• Demonstrate leadership in a professional practice setting under the direction of the RN.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification through AHA or RQI
• Minimum 1 year of experience as an LPN in a Long-Term Care (LTC) facility
• Willingness to work holidays and weekends

Schedule

This position involves a full-time schedule with a D/E rotation, requiring the ability to start on a Tuesday or Thursday for complete shift orientation.

Assignment Duration

This is a 13-week contract position.
