ARMStaffing is looking for a Registered Nurse, Case Manager (Day Shift) in Petaluma, California.

This is a 13-week contract position.

Schedule/Shift

This position requires flexibility with a scheduling pattern that may include 3 days at Petaluma Valley Hospital (PVH) and 2 days at Healdsburg. It includes every other weekend, and may also involve working up to 5 consecutive days based on PTO availability.

Job Description

The Registered Nurse will float between Petaluma Valley Hospital and Healdsburg during this role, providing case management for adult and geriatric populations in an acute care setting. The role emphasizes effective care coordination, discharge planning, and adherence to CMS guidelines while utilizing InterQual criteria. The ideal candidate should demonstrate strong clinical assessments, patient advocacy, and collaborative practices with interdisciplinary teams, including Physical Therapy, Social Services, and Pharmacy.

Responsibilities

• Assess and manage patient admissions and discharges in accordance with established guidelines.
• Coordinate with interdisciplinary teams to ensure optimal patient care and resource utilization.
• Utilize InterQual criteria for admission and discharge decisions.
• Ensure compliance with HIPAA guidelines and CMS regulations.
• Maintain accurate patient records and document care plans using the Epic charting system.

Required Experience

• Minimum of 2 years of acute care nursing experience, with a focus on case management preferred.
• Experience with Epic charting system is required.
• Travel experience is required.

Certifications/Licensure

• Valid Compact RN license must be in hand at submission.
• Additional certifications in case management or related fields are a plus.
