Job Description

Join the team at Powers Health Rehabilitation Center in Crown Point, Indiana, and work in a freestanding inpatient rehab hospital specializing in adult patients, including those recovering from stroke, traumatic/non-traumatic brain injuries, debility, amputation, and neurological disorders such as MS and Parkinson's disease. The facility features 40 beds and utilizes Epic as their EHR system.

Responsibilities

• Conduct evaluations and develop individualized treatment plans for patients.
• Implement therapy interventions and monitor patient progress.
• Collaborate with multidisciplinary team members to optimize patient care.
• Document patient care activities in the EHR accurately.

Required Experience

• Active Physical Therapist license in the state of Indiana.
• 1-2 years of experience in inpatient rehabilitation settings preferred.

Certifications/Licensure

• Licensed Physical Therapist in Indiana.
